AI Governance Evidence Packs: Preparing a Reviewable Record Before Approval

A governance guide for assembling the evidence reviewers need to assess an AI use case without confusing documentation with proof of compliance.

AI review slows down when evidence is scattered across tickets, vendor documents, test results, and informal conversations. An evidence pack gives reviewers a consistent record of what the system is for, what was evaluated, what remains uncertain, and who is responsible for the decision.

Start with context

Describe the intended use, affected users, decision role, data sources, model or service, integrations, human oversight, and consequences of error. Context determines which risks and controls deserve attention.

Include evidence, not just assertions

Link relevant testing, risk assessments, data documentation, security review, vendor information, monitoring plans, incident routes, change controls, and known limitations. Record gaps explicitly instead of filling them with assumptions.

Separate evidence from the decision

The pack supports review but does not approve itself. NIST AI RMF 1.0 organizes AI risk management through Govern, Map, Measure, and Manage and emphasizes documentation and ongoing risk management across the lifecycle.

Keep it reviewable

Use versioning, dates, owners, source links, decision status, conditions, and review triggers. Update the pack when material changes alter the evidence or risk picture.
